Requirements for Passport Application Adults(18 yrs. old and above)

  • Signing of documents to be done personally before a Consular Officer
  • The Philippine Consulate General does not offer PHOTOCOPYING SERVICES.

A. RENEWAL:
     ð Confirmed appointment
     ð Personal appearance
     ð Printed accomplished application form
     ð Current passport with photocopy of data page
     ð Other documents as may be required
     ð Passport fee: RMB456.00 (payment only via bank transfer through online mobile banking)

B. NEW APPLICATION (including renewal of passports issued before 2011):
     ð Confirmed appointment
     ð Personal appearance
     ð Printed accomplished application form
     ð Original and photocopy of PSA-issued birth certificate
     ð Other documents as may be required
     ð Passport fee: RMB456.00 (payment only via bank transfer through online mobile banking)

Additional requirements for special cases:

• For married women who wish to use their married name:
       ð Original and photocopy of PSA-issued marriage certificate OR Report of Marriage (ROM) issued by the Philippine Consulate General in Xiamen. (see list of requirements here) (Note: passport and ROM may be applied for at the same time).
• For previously-married women who wish to revert to maiden name:
       ð Original and photocopy of PSA-issued birth certificate and acceptable IDs

       ð Original and photocopy of annotated PSA-issued marriage certificate OR
  duly authenticated death certificate of spouse
• If applicant is a Naturalized Filipino Citizen
       ð Original and photocopy of Identification Certificate of Naturalization from the
  Bureau of Immigration (BI)
