Requirements for Passport Application for Minors (below 18 yrs. old)

• Signing of documents to be done personally before a Consular Officer
  • The Philippine Consulate General does not offer PHOTOCOPYING SERVICES.

General Requirements:

     •Confirmed appointment
      •Personal appearance of minor applicant and either parent or authorized adult companion Printed accomplished application form
      •Photocopy of Philippine Passport of parent/s
      •Other requirements, as may be required
      • Passport fee: RMB456.00 (payment only via bank transfer through online
mobile banking)
      •If parents are married
      •PSA-issued marriage certificate OR Report of Marriage (ROM) issued by the Philippine Consulate General in Xiamen
• If unaccompanied by either parent
      • Original and photocopy of valid proof of affiliation and/or guardianship of accompanying adult executed by Filipino parent
      •Special Power of Attorney (SPA) executed by Filipino parent in favor of accompanying adult (please see note below).
       •Original and photocopy of passport or Chinese National ID (with English translation written on the photocopy) of accompanying adult

If parents are unmarried
      • If either parent is not accompanying child/children, original and photocopy of valid proof of affiliation and/or guardianship of accompanying adult executed by Filipino mother (if mother is non-Filipino, executed by Filipino father)
      •If Filipino mother is not accompanying child/children, Special Power of Attorney (SPA) executed by the mother (if mother is non-Filipino, executed by Filipino father) (please see note below**)
      •Original and photocopy of passport or Chinese National ID (with English translation written on the photocopy) of accompanying adult

A. For new passport (including renewal of passports issued before 2011; ROB/ROM and new passport may be applied for at the same time):

      •Original and photocopy of PSA-issued birth certificate OR Report of Birth issued by the Philippine Consulate General in Xiamen (see list of requirements here)
      •Original and photocopy of PSA-issued marriage certificate OR ROM issued by the Philippine Consulate General in Xiamen (see list of requirements here)

B. For renewal of passport
       Current passport with photocopy of data page
       Proof of filiation (any one of the following documents: PSA-issued birth certificate; baptismal certificate; DSWD travel clearance)

•  **For SPAs:
  If executed within Fujian and Jiangxi, notarized and authenticated by Fujian or Jiangxi FAO
  If executed in the Philippines, apostillized/authenticated by the Department of Foreign Affairs (DFA)
  If executed outside the Philippines and outside Fujian and Jiangxi provinces, authenticated/acknowledged by the relevant Philippine Embassy or Consulate General
