Requirements for LOST, MUTILATED/DAMAGED PASSPORTS

  • The Philippine Consulate General does not offer PHOTOCOPYING SERVICES.

     ð Confirmed appointment
     ð Personal appearance
     ð Printed accomplished application form
     ð Copy of old passport, if available
     ð Original and photocopy of PSA-issued birth certificate
     ð For lost passports:
          ð Affidavit of loss of passport (download form here)
          ð Police report (notarized and authenticated by Fujian/Jiangxi FAO)
     ð For mutilated/damaged passport, Affidavit of mutilated/damaged passport (download form here)
     ð All other requirements for new passports (adult and minor)
     ð Other documents as may be required
     ð Fees: RMB1,330.00 (payment only via bank transfer through online
mobile banking; other notarial fees may be added)

*Additional 15 calendar days clearing period is required for Lost Valid Passports.
