Requirements for 9(c) Crewlist visa filed by an Authorized Agent in Xiamen

• To be filed by licensed manning agency (LMA), agent, or authorized representative of shipping company in Xiamen.

  • Please accomplish the online form completely to ensure that your application will be accepted.

  • At the designated date, please bring all requirements in triplicate:
  1. Ship Registry Certificate
  2. Authorization from the owner of the vessel authorizing the licensed manning agency (LMA), the ship’s agent, or their authorized representative at the Consulate General (please include information on the shipping agent in Manila)
  3. Notarized Affidavit of Support and Guaranty executed by the authorized personnel of the shipping company in the Philippines citing their responsibilities as the shipping agent and providing information of the seafarers' flight details/itinerary;

Duly accomplished forms:
4. FA Form no. 2 (for each applicant)
5. FA Form no. 61 (please ensure that signature of Master/Captain is affixed in part F)

6. Fees (payment only via bank transfer through online mobile banking)
