Requirements for 9(c) Seafarers' Visa (individual)

• By appointment only. Please call 0592-513-0355/66.

1. Duly accomplished application form. Please download here;
2. Original and photocopy of passport valid for at least six (6) months;
3. Original and photocopy of valid seaman’s book;
4. Certificate of employment with the following information:
ü seaman’s personal details and schedule in joining the vessel in the Philippines;
ü name/call sign of the vessel in the Philippines
ü Philippine shipping agent in the Philippines with its contact person, address and contact numbers.
5. Notarized Affidavit of Support and Guaranty executed by the authorized personnel of the shipping company in the Philippines citing their responsibilities as the shipping agent and providing information of the seaman’s flight details/itinerary;
6. Confirmed plane ticket
7. Fees – RMB342.00 (payment only via bank transfer through online mobile banking)
