CONVERSION TO 9F VISA CHECKLIST
Requirements: 所需手续
- Personal Appearance申请人本人到场
- Police Clearance issued by authorities from place of origin/residence
由原籍地/居住地公安机关出具的无犯罪记录证明
- Medical Examination for Visa Application (FA FORM no. 11, please click here) issued
by a licensed physician
由持照医师签署的签证申请体检表（FA表格编号11，请点击此处下载）
- Scholastic Records 学籍记录
- Affidavit of Support (for students financially dependent on their parents)
经济担保书（经济上依赖父母资助的学生须提供）
NOTE: Nos. 2-5 above are to be authenticated by the Philippine Consulate General in Xiamen. It must be duly notarized by the local notary public, translated to English and authenticated by the Fujian/Jiangxi Foreign Affairs Office (FAO).
注:上述第2-5项材料须双认证，经当地公证处公证，翻译成英文，再经福建/江西外事办(FAO)认证并递交至菲律宾驻厦门总领事馆认证。
Documents (please prepare in the following order):申请材料（请按以下顺序整理）：
Original and photocopy Notice of Acceptance (NOA) from School or University (with
dry seal and signed by the authorized signatory) for the current School Year
录取通知书原件和复印件 （盖有学校钢印并由授权签字人签署）
- Duly accomplished visa application form (FA Form no. 2) - downloadable at the
Consulate General website (xiamenpcg.dfa.gov.ph) 填写完整的签证申请表 (FA Form
- 2) – 此表可在总领馆官网(xiamenpcg.dfa.gov.ph) 下载
- Information details match passport 填写内容与护照信息一致
- Paste passport-size photo (taken within 6 months, white background)
粘贴护照标准规格的照片（最近6个月拍摄的白底彩照）
- Name, address and telephone number of school as sponsor/reference
担保人信息栏要填写录取学校的名称、地址和电话
- Latest Visa No. (if applicable) 最近的签证号（如果有申请过）
- Duration of last stay in the Philippines (if applicable)
最近一次在菲律宾的停留时长（如果有去过的话）
- Signed 本人签名
- Original passport valid for at least six (6) months beyond the authorized period of stay
in the Philippines, and with at least two (2) blank pages
护照/旅行证件的原件和复印件，有效期须超出在菲律宾停留期限外至少6个月且留有

- Clear photocopy of passport data page 护照信息页清晰的复印件
- Original Chinese National ID 中国公民身份证原件

copies of the latest Philippine visa and immigration arrival and departure stamps

g. Photocopy of the company’s business license in China (with QR code) with English translation 中国公司（有二维码的）营业执照复印件并有英文
