GENERAL REQUIREMENTS FOR ALL NOTARIAL SERVICES:
○ Confirmed appointment
○ Personal appearance
○ Original and photocopy of valid passport data page and/or Identification Card with English translation
○ Unsigned document/s to be notarized
○ Photocopy of unsigned document/s
○ Processing Fees - (payment only via bank transfer through online mobile banking) RMB266.00 per document/per service
REQUIREMENTS FOR SPECIFIC SERVICES:
1. AFFIDAVITS
An Affidavit is a written statement under oath. (Please download forms here)
Examples:
- General Affidavit
- Affidavit of Support and Consent (for application of passport and/or travel of child)
○ Confirmed appointment
○ Unsigned accomplished Affidavit Form/s
○ Original and photocopy of valid passport data page and/or Identification Card
○ Other supporting documents as attachments
○ Photocopy of Affidavit/s
2. ACKNOWLEDGEMENT
An Acknowledgement is the act by which a person who has executed an instrument goes before a competent officer and declares it to be his true and voluntary act and deed.

- Deeds of Sale
- Real Estate Mortgages
- Other documents

For business contracts wherein a party in China is a Corporation:
- Business Registration with English translation, notarized and apostilled by Fujian or Jiangxi Foreign Affairs Office
- Proof of authority for the appointed person to sign the contract with the following attachments:
- Notarized Authorization letter with English translation and apostilled by Fujian or Jiangxi Foreign Affairs Office
- ID of a person authorized to sign the contract (e.g. passport or Chinese national ID with English translation)
- ID of authorizing person (e.g. passport or Chinese national ID with English translation)
- Other documents as may be required
3. NBI FORM NO. 5 AND FINGERPRINTING
Processing and release of the NBI Clearance Certificate may only be done in the Philippines. The NBI clearance applicant in China may authorize a representative, through a Special Power of Attorney (SPA), to submit an NBI Form No. 5 and claim an NBI Clearance Certificate.

○ NBI Form No. 5 (available at Xiamen PCG)
○ 2 pcs. photos (2”x2” with white background taken within 3 months prior to application)
