1. REPORT OF BIRTH (ROB)
The birth of a Filipino child in Fujian and Jiangxi must be reported to the Philippine Consulate General in Xiamen within one (1) year after birth of child.
General Requirements:
- Confirmed appointment
- Personal appearance of parent/s
- 4 sets of ALL documents
- Printed duly accomplished Report of Birth Form, all originals (download form here)
- Valid passports or national I.D. with English translation of parents (original and 4 photocopies)
- Birth Certificate with English Translation, notarized and apostilled by Fujian or Jiangxi Foreign Affairs Office (original and 3 photocopies)
- Other documents as may be required
Additional requirements for special cases (1 original and 3 photocopies):
For married parents:
- Marriage Certificate
o If married in the Philippines, PSA-issued Marriage Certificate and apostilled by the Department of Foreign Affairs
o If married in China, Marriage Certificate with English translation, notarized and apostilled by Fujian or Jiangxi Foreign Affairs Office or Report of Marriage issued by the Philippine Consulate General in Xiamen
For unmarried parents:
- Affidavit to Use the Surname of Father (AUSF), if applicable
- Affidavit of Admission of Paternity (AAP), if applicable
For Delayed Registration of Birth
- Affidavit for Delayed Registration of Birth
2. REPORT OF MARRIAGE (ROM)
The Marriage of a Filipino within Fujian and Jiangxi provinces should be reported to the Philippine Consulate General in Xiamen within one (1) year after the solemnization of marriage.
General Requirements:
- Confirmed appointment
- Personal appearance of a couple
- 4 sets of ALL documents
- Printed duly accomplished Report of Marriage (ROM) Form, all originals (download form here)
- Valid passport or Identification Card of husband and wife (present original and 4 photocopies)
- Chinese-issued Marriage Certificate with English translation, notarized and apostilled by Fujian or Jiangxi Foreign Affairs Office (original and 3 photocopies), except if marriage is solemnized at the Consulate General
- 4 photocopies of PSA Birth Certificate
- 4 photocopies of the Hukou with English translation, reflecting details of parents if the partner is Chinese
Special requirements for special cases (original and 3 photocopies):
If one or both parties was/were previously married:
• For annulled Filipino spouse - marriage contract issued by the PSA with proper annotation and apostilled by the Department of Foreign Affairs
• For divorced Filipino spouse - marriage contract issued by the PSA with proper annotation and apostilled by the Department of Foreign Affairs
• For widowed Filipino spouse - PSA-issued marriage contract with previous spouse AND PSA-issued death certificate apostilled by the Department of Foreign Affairs or foreign death certificate of previous spouse (in English) apostilled
• For divorced or annulled foreign spouse - foreign decree/decision (in English) notarized and apostilled
• For widowed foreign spouse - death certificate (in English) of previous spouse notarized and apostilled

3. REPORT OF DEATH (ROD)
General Requirements:
- Printed duly accomplished Report of Death (ROD) Form, all originals (download form here)
- Passport or Identification Card of the deceased (original and 3 photocopies)
- Death Certificate with English translation, notarized and apostilled by the Fujian or Jiangxi Foreign Affairs Office (original and 3 photocopies)
- Other documents as may be required
