Philippine Passport Application Form

To the Filipino Community in Fujian and Jiangxi Province:

The Philippine Consulate General in Xiamen would like to inform that the Department of Foreign Affairs (DFA) is strictly reminding all Passport applicants to completely accomplish Passport Application Forms upon application.

  When applying for the issuance or renewal of a Philippine passport with the Philippine Consulate General in Xiamen, please ensure that all fields indicated in the passport application form are accomplished by the applicant. It should be remembered that all information declared in the application form must be complete, correct and accurate prior to the issuance of Official Receipts, data encoding and capturing of  the applicants’ biometrics (photo, fingerprints and signature).
