   Printed and signed OVAS application form

   Original and photocopy of passport valid for at least six (6) months beyond the authorized period of stay in the Philippines, and with at least two (2) blank pages

SOLEMNIZATION OF MARRIAGE

Marriage between two (2) Filipino nationals, male and female, who are at least eighteen (18) years of age at the time of marriage, may be solemnized at the Philippine Consulate General.
